This will include finding out which countries and areas within countries you are visiting to determine what vaccinations are required. Complete the form below and send it to us online or bring it into the surgery at least 8 weeks before you travel (the vaccines have to be ordered specifically and are not kept in stock. They also have to have sufficient time to work once you have been vaccinated).  Reception will then contact you to make an appointment.  Please give us as much notice as you can - at least 8 weeks before you travel if possible -

If we do not receive 8 weeks notice there are travel clinics are available in Derby and Nottingham  http://www.travel-doc.com/    https://www.masta-travel-health.com/

There is further information about countries and vaccinations required on the links below:

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.  Please speak to a member of our team for guidance on the costs of vaccines.

Travel Health Questionnaire

To help us offer the appropriate advice, please read the guidelines below and then complete the form and bring it into the surgery.   A nurse will then contact you to arrange an appointment.  Please give us as much notice as possible.
